James Reason Swiss Cheese in VR

An educational tool for explaining James Reason's Swiss Cheese model to students new to aviation.

Aerospace Australia in partnership with Swinburne University created a series of microcredentials to teach safety basics to students in the Aviation industry. Lightning Rock was contracted to develop a series of VR training simulations to complement students learning in certain subjects of this course.

Given the significant consequences of failure a good understanding of risk management is essential. Swiss Cheese VR creates an interactive and gamified simulation based on the work of James Reason. It explores how nearly all major incidents result from a series of failures in different areas of control.

Using the Oculus Quest VR headset, players are put in the middle of a laboratory and given a set amount of resources which they can use to mitigate risks that lead to a set risk. The simulation was created in a way that additional scenarios can be quickly and easily added based on the customers requirements.

Swiss Cheese was created with the support of an expert in aviation risk management to ensure its accuracy and authenticity. It is currently being used by students of the MyFlightPath safety microcredential course.
